GMS Athletics Information
Welcome to Greenacres Middle School, where students can get involved in our athletic programs.
STEP 1: Parents AND athletes must complete the Final Forms online registration.
Student Final Form accounts must use their student email. (Student ID#@cvsd356.org) *Please refer to help guide for creating accounts.
STEP 2: Provide and upload a current physical in Final Forms. Physicals are good for two years and are required for any participation.

- $20 ASB Fee (paid each school year) -
- In addition, each sport has a $10 activity fee and a $5 participation fee. ($15 Total per Sport)
- Fees must be paid to participate in a contest.

Note: If your student qualifies for free or reduced lunch they may be eligible for a sports fee waiver. Please contact office for more information.
STEP 4: Proof of Insurance. All students participating in athletics must be covered by medical insurance with information verified on Final Forms.
*Please note that student-athletes are not eligible to participate until they have completed the above steps and been approved by the Athletic Administration.
Central Valley Middle Schools operate no-cut athletic programs. However, this does not mean that students will have equal playing time. When we have large turnouts, we create one or two additional "junior varsity" teams to allow for more playing time for student-athletes.
